Approximate claim processing time for insured claims is six (6) weeks. Please follow all instructions provided when completing this form. DHL must receive written notification of claims within 30 days from the date that DHL accepted the Shipment.

This form is subordinate to the terms and conditions of the Shippers Interest Insurance (“SII”) policy. The SII policy, in turn, is subordinate to DHL’s terms and conditions of carriage. In accordance with DHL’s terms and conditions of carriage, DHL’s liability for uninsured shipments is limited to its actual cash value and shall not exceed:

$US 30.00/kilogram or $US 13.61/lb for Shipments transported by air or other non-road mode of transportation; or $US 11.00/kilogram or $US 4.99/lb for Shipments transported by road.

DHL is not liable for delay or consequential damages under any circumstances.
